Family Support Worker

Location: CPLC Parenting Arizona-Tucson
Family Support Worker (FSW) are responsible for direct delivery of social services to the children and families of Arizona. The Building Resiliency Program provides in-home support services for families that are referred after case closure from contractors. The FSW will provide advocacy and support services information to families about community resources and assists them in accessing appropriate services, as well as providing skills training to the parent/child.
· High School degree, GED or higher education in any field and have at least one year of experience in Social Work, Psychology, or in any Human Service Field working with children, families, and or adults, as verified on a resume and reference verification
· Ability to travel statewide
· Capable of working with diverse cultural populations
· Knowledge of and ability to access community resources
· Knowledge and understanding of family dynamics and family centered services
· Experience in cultural diversity, substance abuse, impact of child abuse and neglect on child development, sexual abuse, family violence and other interventions
- Knowledge of infant and child development and parent-child attachment, mental health and the dynamics of child abuse and neglect
- Knowledge and experience in strength-based and family-centered provision of primary prevention services
- Possess excellent written and verbal communication skills.
- Must be able to pass a drug screening every 6 months
- Must be at least 21 years of age and must have a valid driver’s license
· Must have a valid Arizona Level One Fingerprint Clearance card (copy will be made at interview)
· Must be able to pass Department of Child Safety Central Registry Background Check
· Bilingual (English and Spanish) required
· Knowledge and experience in In-home visitation
· Develop and work with clients and their families referred to program by funding source.
· Travel to:
ð Travel to client’s home
ð Travel to community locations as needed
ð Travel to take client to schedule appointments, services as necessary when deemed appropriate by Family Support Worker in keeping with the service plan
· It is the responsibility of the Family Support Worker to
ÿ To make decisions in regard to treatment plan/process; i.e. curriculum, resources, and skills training
ÿ To conduct home visits based on the family’s discretion; in regards to times and dates that accommodate the family, regardless of time, day, and location. In addition, be responsible in meeting with families on a regular basis as determined by contract requirements in the Scope of Work.
ÿ To implement treatment processes with the families they are serving
ÿ To conduct Intake/Needs Assessments based on needs of the family and be responsible for deciding admission scores, progress and discharge scores based on the funder’s Scope of Work
ÿ To decide over all if a family has successfully completed services and rate the percentage of services complete.
· Provide appropriate and timely services as indicated in the service plan and optional services as agreed upon by agency and contractor
· Demonstrate respect for client and children in a non-judgmental manner and will respect client’s rights to privacy and confidentially.
· Monitor and document the physical and emotional health of children during visits and assist parents in demonstrating how to care for them.
· To determine the safety with caregiver and others living in the household, to make a decision if a Child Abuse Report needs to be called into the DCS hotline due to any suspected abuse, neglect, and/or safety concern.
· Attend regular meetings with supervisor to review case history and implement service plan; attend staff meetings and staffing as requested
· Maintain accurate and timely written reports, summaries and contact logs.
1. FSW must document all interaction between parents and children on their case notes
2. Monthly Progress Case Notes & Closing Reports are due the supervisor on the 1st day of the month
3. Monthly billing sheets providing family accurate details including the service hours, referrals, visits and services completed
· Must have access to safe transportation with appropriate Arizona drivers’ license and automobile insurance as required by State of Arizona.
· Adhere to all contractual agreements between PA and funding sources and adhere to agency policies and procedures
· Conduct client assessments to assess the risk factors for the family and decide what curriculum is appropriate based on family need
- Knowledge of community resources and ability to refer families to appropriate services, including those offered by Parenting Arizona and CPLC.
- When subpoenaed, testifying in court regarding a case according to PA policy.
· Attend special meetings, trainings, workshops, conferences or join committees related to job as requested by supervisor or executive director.
· Carry out activities in annual work plan and agency’s strategic plan.
· Ability to travel statewide as needed
· Other duties as requested by Program Manager
It is expected to participate in continuous learning, competency building and maintenance of competency skills. A minimum of 20 hours per Fiscal Year including but not limited to Relias Learning, Conference, Workshops, and/or Outside Training.
- In home visitation between parents and children in the household
o Visits are to be conducted in home or designated place by the family.
o All families are required to have a home visit at least two times each month per the funder.
o FSW may be required to transport Parents and Children to appointments that relate to focused goals
- Must be flexible to schedule changes based on family and funder needs
- Must be available to work early mornings, days, evenings and weekends as needed
- Home visits occur based on the family’s availability and times that are convenient for them to meet.
- Must be available to travel throughout Arizona as needed with program requirements.
Reports to and is supervised directly by the Program Manager. Employee will be:
ð Accountable for understanding and complying with CPLC, DCS and any other grant/contract policies and procedures.
ð Meeting all documentation deadlines.
ð Reporting any incidents and/or emergencies to their direct supervisor within 12 hours of the incident.
